Health and Welfare Benefits

Associates scheduled to work 30 or more hours per week that are classified as full-time are eligible for most benefit plans.
The list below provides an overview of the main benefits offered by Warren Distribution.  You can see more details on these plans by continuing to scroll down the page.  

For Your Health

All full-time associates are eligible at the beginning of the month after one full month of employment. Our medical plan features a preferred provider organization (PPO). If you utilize members of the PPO, you will receive greater benefits. Generally, in-network physician office visits are covered in full after you pay a $25 copayment; Specialists are a $40 copayment; TeleHealth services are a $15 copayment. In-network deductible for other plan expenses is $1,250 for individual and $2,500 for family.

Coverage includes tiered prescription drug coverage (Generics $10 or 20%, Brand $15 or 20%, Non-preferred Brand $40 or 20%) Prescription coverage also includes a mail order prescription drug option. Contribution costs are determined based on the level of coverage you select and your smoking status/ wellness participation. Premiums are deducted on a pre-tax basis.

All eligible associates may elect dental insurance coverage. Preventative services, such as exams and cleanings, are covered twice per calendar year at 100% with no deductible. Basic services such as fillings are covered at 80% after the deductible ($50 per individual, $150 per family). Major services, such as crowns and dentures, are covered at 50% after the deductible. Orthodontic services are paid at 50% after the deductible up to $2,000 lifetime maximum per person. Contributions are determined based on the level of coverage selected and are deducted on a pre-tax basis.

Associates may purchase this voluntary program to meet their vision care needs. Generally, spectacle lenses are covered in full after $15 copay. Frames are covered for $15 copay (up to $100-$150). Contacts are covered for $15 copay up to $130 (in lieu of frame and spectacle lenses). Annual eye exams are $15 copay.

The Warren Distribution Wellness Program saves you over $500 per year on your health coverage and over $1,000 if you and your spouse participate.

For Your Wealth

Associates may purchase this voluntary program. Benefits are payable after being disabled for 14 days due to accident or sickness. Benefit is 60% of associate’s weekly salary, with a maximum benefit of $1,500 per week, payable for up to 180 days based on disability.

We pay 100% of the premiums for coverage of 1 times your base pay, rounded to the next higher $1,000, with a minimum of $20,000 and a maximum of $250,000. The coverage automatically includes Accidental Death and Dismemberment coverage.

Warren’s Short-Term Disability Insurance through Mutual of Omaha allows associates to elect income protection coverage in the event that you are not able to work due to an illness or injury for more than 14 days for up to 24 weeks.

We pay 100% of the premium for LTD for eligible associates.

An associate who has become disabled may receive, after a six month qualifying period, a benefit of 60% of the associate’s monthly earnings. The maximum monthly LTD benefit is $12,500.

Full-time non-exempt associates may purchase Voluntary Long Term Disability through payroll deduction. The benefit begins 180 days after a qualifying disability.

BestCare EAP provides support and guidance for matters that range from personal issues you might be facing, to providing information on everyday topics that affect your life.

Eligible associates may establish a pre-tax medical spending account up to $2,600 annually and/or dependent care account up to $5,000 annually.

Associates are eligible to participate at the beginning of the month after one full month of employment. Associates can contribute up to 100% (subject to IRS limitations) of base pay to the plan on a pre-tax or post-tax basis. For every dollar an associate contributes, the Company will match it with 50 cents-up to the first 6% of associate savings. Associates direct their contributions in an array of investment funds and are 100% vested in the company match after 3 years of employment.

For Your Personal Life

Eligible associates will accrue PTO pay based on their length of service:

0-4                              10 days

5-9                               15 days

10-19                          20 days

20+                             25 days

Associates will also accrue 2 days (annually) of CAT to cover illness or disability lasting (2) weeks or longer. Funeral leave of (3) days is provided for the death of an immediate family member.

Pre-approved educational costs for tuition and fees will be reimbursed at 100% for a grade “C” or above, up to a maximum of $2,000 annually.

A Merit Scholarship of $1000 (one for each college class level) is awarded annually to the child of an employee who demonstrates academic ability. A General Scholarship of $100 is given to the children of associates who graduate from high school and have enrolled in an accredited college or voc/technical school, that do not receive the Merit Scholarship.
